1.  Services Coordination: The Service Coordinator helps potential participants navigate the waiver enrollment process and oversees the delivery of all services detailed in their Service Plan.

2. Community Integration Counseling (CIC): CIC provides personalized support to help waiver participants thrive emotionally as they adjust to community living. Through counseling, participants can address changes in their abilities, revise their future expectations, and navigate evolving relationships with those closest to them.

3. Home and Community Support Services (HCSS): Home and Community Support Services (HCSS) are designed to ensure the health and welfare of participants living in the community, primarily when oversight or supervision is needed. These services can also include personal care assistance with Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL).

4. Independent Living Skills Training Services (ILST): The ILST provider must assess the waiver participant's functional abilities comprehensively, determining their strengths and weaknesses in performing Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) while considering their established goals.

5. Positive Behavioral Intervention and Support Services (PBIS): PBIS focuses on reducing problematic behaviors and developing more socially appropriate responses.

6. Structured Day Program: The program is tailored to meet each participant's unique needs, offering essential services in outpatient group settings or the community. Our main goal is to empower individuals to achieve greater independence by enhancing their functional skills.

What are the Eligibility Requirements for the NHTD/TBI Waiver Program?

The New York State Nursing Home Transition and Diversion (NHTD) & TBI Waiver program has specific eligibility criteria to guarantee that participants significantly gain from community-based living and assistance. These criteria cover both disability and financial aspects, with the following primary requirements:

  • Must have an active Medicaid coverage​

  • Age Requirement: Must be between 18 and 64 with a verified physical disability or 65 or older when applying to the program. 

  • Have a physical disability or traumatic brain injury (TBI) resulting in the need for a level of care typically found in a nursing home.  â€‹

  • The Ability to Live in the Community.

Generally, if an applicant already has a residence, adequate informal support, and a designated Service Coordinator, enrollment typically takes about 3-4 months.
