What is an NHTD Waiver?

The Nursing Home Transition and Diversion (NHTD) waiver is a Medicaid program designed to help eligible individuals with disabilities or chronic illnesses transition from nursing homes or avoid institutionalization. It provides tailored services and supports that promote independent living and active participation in community life.

What is a TBI Waiver?

A Traumatic Brain Injury (TBI) waiver is a state-administered Medicaid program that supports individuals living with the effects of a brain injury. It offers specialized care, rehabilitation, and resources not typically covered by traditional insurance, helping participants live as independently as possible and maintain quality of life.

The TBI waiver addresses a wide range of needs — from daily living assistance to long-term support — carefully tailored to meet the complex and unique challenges faced by individuals recovering from brain injuries. Without these waivers, many would struggle to access essential services for rehabilitation, daily living, and overall well-being.

Independent Living Skills Training (ILST)

Home and Community Support Services (HCSS)

Ensures participants’ health and safety at home, including personal care assistance with Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL).

Eligibility

Requirements

To qualify for the NHTD/TBI Waiver Program in New York State, applicants must:

Have active Medicaid coverage

Be 18–64 years old with a verified physical disability, OR 65+ when applying

Have a physical disability or TBI requiring a level of care typically provided in a nursing home
